Strong Start

  Exciting news: REACHUP, Inc. and its’ Central Hillsborough Healthy Start Program is happy
to announce our partnership with the FL Association of Healthy Start Coalitions in a successful
application to the Strong Start for Mothers and Newborns Initiative, a funding opportunity
offered through the Center for Medicare & Medicaid Innovation (CMS Innovation Center). We
are excited about our Enhanced Care Model role which includes medical, psychosocial and behavioral
interventions.

  The Florida Association of Healthy Start Coalitions was recently awarded this Strong Start for
Mothers and Newborns grant to implement Maternity Medial Homes in three Florida counties
(Hillsborough, Pinellas and Polk) through five different prenatal health care provider offices (health
department, Federally Qualified Health Center, private OB office, private Medicaid clinic and
regional perinatal intensive care center). The state Healthy Start Coalition of Hillsborough
County, Inc. serves as the Project Lead for the state. REACHUP, Inc. is part of this grant
application through its work over past 14 years with one of the federal HS prenatal
provider offices Exodus Clinic. This application will implement the Maternity
Medical Home (MMH model) in these counties to reduce preterm births and improve equities in this
outcome in this region. This project serves as a pilot for the state of Florida in preparation for
Medicaid reform and the redesign of state Florida Healthy Start services.

 Grant states client will receive:

  • Face to face visits in the OB office
  • Screens for DV, substance abuse and misuse and mental health concerns
  • HROB Case Management
  • Chronic Disease Management
  • Explanation of medical regime
  • Assistance with procuring maternal/fetal testing/medications
  • Referrals to community resources
  • Family planning, birth plans
  • schedule postpartum visit and follow up to make sure it is kept
